How To Register

NEW!  Fillable PDF Registration Form……OR Regular PDF to print,  fill out and send in by scan/email,  fax or regular mail.

  1. Choose the program(s) from our home page that your child/children will be attending and take note of the course code(s).
  2. Be sure to state whether or not your child will take the OPTIONAL LUNCH PROGRAM. Also, please indicate whether your child is attending as a PLAYER or a GOALIE.
  3. There is a sibling discount of 10% for fall/winter classes and all camps. Take 10% off the price of the camp before adding HST.
  4. Sibling discount does not apply for Spring 4 on 4 Program or specialty classes such as Select Pre -Tryout Classes.
  5. Carefully read all CPHC Policies and Waiver before signing. PLEASE NOTE THAT THE REGISTRATION FORM IS NOT VALID WITHOUT A SIGNED WAIVER.
  6. Send in the registration form with payment. We accept Cheques, Visa or MasterCard or Cash. Forms can be scanned & emailed, faxed into our office at 416-322-3383 or mailed in to our office or dropped off in person. WE DO NOT ACCEPT REGISTRATION OVER THE PHONE.
    A written confirmation letter will be emailed out within 2 weeks of receiving your form. If you do not receive a confirmation letter within 1 week, please contact our office.
